Counseling visits are 45 to 50 minutes. At times, visits may be slightly longer.

For people without behavioral health insurance or those not wishing to use their insurance, my fee is $140.00 per visit. Payment can be made in cash, check, and debit or credit card. I will provide a statement you can supply to your Health Savings Account for reimbursement. Please let me know, before the visit, if you want such a statement.

INSURANCE
I am a provider for the following insurance companies, and I accept their payment in full (plus your copay if applicable).

United Healthcare/Optum, Blue Shield, CIGNA, Aetna, Magellan, MHN,  Sutter EAP Services, and CONCERN EAP. I bill these insurance companies myself.

I will accept Anthem Blue Cross PPO, although I am not a contracted provider. I will bill Anthem for you, if you wish. I can also supply a statement for you to provide to the insurance yourself.

Insurance companies come and go, divide, rename, etc.  If your insurance is not listed above, check with your insurance company , I may be a provider.

As a provider for these insurance companies, I accept the fee they approve as payment in full. The client is responsible for their yearly deductible, and any co-pay their plan includes.

REDUCED FEE
A reduced fee slot may be available. Reduced fee slots, are times when the majority of people are unable to attend counseling. These times are typically mornings during the work week. Reduced fee slots are for people in financial need. Feel free to enquire about reduced fee and reduced fee slots.

PAYMENT
Payment for your visit or your copay is due at the beginning of each visit at each visit. I accept payment in check, cash, credit or debit cards.

CANCELLATION POLICY
I have a 24-hour cancellation policy. If you fail to cancel 24 hours prior to your visit, you may be charged the FULL FEE for that visit.
